Dog Adoption Application
Please answer the questions below completely and honestly. We strive to make good matches between adopters and pets, find lifelong companions for our clients, and place animals in loving and responsible homes.
What’s Included?
The adoption fee for our dogs and puppies includes all of the following:
- Spay/neuter surgery
- Deworming
- Heartworm test
- Parasite preventative
- Bordetella, distemper, and rabies vaccination (if age-appropriate)
- Microchip
- Certificate of Veterinary Inspection (required by the state of Wisconsin)
- Certificate for a free first vet exam (at participating clinics)
If you were to do all these procedures yourself the average cost would be upwards of $300!
